Need a companion?

PA Connecting Communities offers a variety of opportunities using a companion for individuals that need special supports to live at home and within the community as independent as possible. All programs and events offered by PA Connecting Communities are designed to provide home and community habilitation services. All activities teach the skills needed to integrate into the community, maximizing the potential to live as independent as possible with outcomes that can be measured and achieved. These programs are planned for individuals who require assistance in the acquisition, retention, or improvement of skills related to living and working in the community.

We offer services, at multiple levels of staff to participant ratio for individuals or small groups, or by staff with specialized training.  We offer 1:1, 1:3, 1:6 ratios for waiver funding billing.   Services are planned in accordance of need established through the assessment processes in the participant’s IEP or ISP.

PA Connecting Communities follows the principles of self-determination.  All programs and events are consistent with our Mission and are directed towards community inclusion.  We value the rights of all individuals and emphasize the strengths of each person while developing areas of need.

Contact your supports coordinator and our office for consideration of waiver funding guidelines.  As always, programs may be self-paid and special arrangements may be made according to the individual’s needs. (412) 784-1470.
