Programs are
offered day,
extended-day,
evening and
weekends; where
the need is the
greatest.
PA
Connecting Communities
offers a variety of
opportunities to
individuals that need
special supports to live
at home and within the
community as independent
as possible. All
programs and events
offered by PA Connecting
Communities are designed
to provide CHOICE. All
activities teach the
skills needed to
integrate into the
community, maximizing
ones potential to live
as independent as
possible with outcomes
that can be measured and
achieved. These programs
are planned for
individuals who require
assistance in the
acquisition, retention,
or improvement of skills
related to living and
working in the
community.
In
recognition of
requirements to ensure
individuals’ health and
welfare, we offer
services, at multiple
levels of staff to
participant ratio for
individuals or small
groups, or by staff with
specialized training. We
offer 1:1, 1:3, 1:6 and
1:12 ratios for waiver
funding billing.
We
provide programs within
the community, at
various community
centers, places of
service such as fitness
centers and/or at group
homes for your
convenience. Programs
and events are planned
in accordance to the
need for services
established through the
assessment processes in
the participant’s IEP or
ISP.
